Join Mascoma Audio Visual

Welcome! Mascoma Audio Visual is a program open to students 14 years and older who are interested in audiovisual projects. Meetings take place Thursdays 3-5 at Mascoma Valley Regional High School, but could be changed for individual projects.

By filling out this application, you acknowledge and agree to the following:

Eligibility & Consent:

• Students under 18 require parent/guardian consent to participate.
• Students 16+ may be eligible for paid opportunities; completing a W-9 and parental consent is required.
   

Purpose of Information:

• To enroll students in the Mascoma Audio Visual program.
• To ensure safety, obtain parental consent, and facilitate participation in program activities.
• To manage potential paid opportunities for students 16 and older in compliance with tax requirements.

Confidentiality:

• Your personal information will be kept private and used only for program-related purposes.
   

Agreement:

• By submitting this application, you (and your parent/guardian, if required) acknowledge that you have read, understood, and agreed to these terms.
• By submitting this application, you and your parent/guardian consent to the use of your student’s images, video, and audio recordings for Mascoma Audio Visual activities and promotional purposes.
• By submitting this application, you acknowledge that participation may involve risks typical of audiovisual activities, and you agree to the terms of the Hold Harmless Agreement.
• By submitting this application, you affirm that all information provided is accurate and complete to the best of your knowledge.
• By submitting this application, you and your parent/guardian agree to follow all program rules and policies established by Mascoma Audio Visual.
• By submitting this application, I acknowledge that this constitutes a legal electronic signature and agreement to the above terms.
